Permanency Facilitator 

  • Family Pathways is hiring a full-time Permanency Facilitator. In this position, you will be working with children who are in out-of-home care (foster care) and foster families. The responsibilities of this position include implementing SWAN services to assist the children and families in understanding the foster care process while using creativity to engage the children. This position requires bi-weekly visits to the home, court hearings, composing monthly reports, and maintaining communication with the clientele and referral sources. In this position, you will follow both the state certifications and the SWAN training requirements, participate in recruitment events, and conduct biological family visitation.

  • Good written and oral communication, time management, thorough documentation, creativity, and computer skills are necessary for this position.

  • This position requires a Bachelor’s Degree in a Social Service related field or a minimum of two years’ experience working directly with children with an unrelated Bachelor’s Degree. Evening hours and travel are required. Upon hire, applicants must obtain a physical, required clearances (FBI, Act 33 and Act 34), provide a valid driver’s license and current car insurance.

Placement Facilitator 

  • Family Pathways is hiring a full time Placement Facilitator. In this position, you will be working with children who are in out of home care [foster care] and foster families. The responsibilities of this position are case management oriented focusing on the safety and well-being of the children. You will conduct monthly home visits, attend court hearings, compose monthly reports for each child, and maintain communication with clientele and referral sources. In this position, you will facilitate foster and kinship state certifications, participate in recruitment events, and conduct biological family visitation. You will be cross-trained in the Legacy Program which works with the biological families.

  • Good written and oral communication, time management, thorough documentation, and computer skills are necessary for this position.

  • This position requires a Bachelor Degree in a Social Service related field or a minimum of two years experience working directly with children with an unrelated Bachelor’s Degree. Evening hours and travel are necessary.  Upon hire, applicants must obtain a physical, required clearances (FBI, Act 33, and Act 34), and provide valid driver’s license and current car insurance.

       

Legacy Facilitator 

  • Family Pathways is hiring a full time Legacy Facilitator. In this position, you will be working with families to maintain or achieve reunification with their children. The responsibilities of this position are case management oriented focusing on the safety and well-being of the family. You will conduct home visits, attend court hearings, compose monthly reports for each client, and maintain communication with referral sources. In this position, you will provide support to the biological family in meeting the goals on their individual service plans.

  • Good written and oral communication, time management, thorough documentation, and computer skills are necessary for this position.

  • This position requires a Bachelor Degree in a Social Service related field or a minimum of two years of experience working directly with children with an unrelated Bachelor’s Degree. Evening hours and travel are necessary.  Upon hire, applicants must obtain a physical, required clearances (FBI, Act 33, and Act 34), and provide valid driver’s license and current car insurance.

Parent-Child Visitation Facilitator 

  • Family Pathways is hiring a full time Parent-Child Visitation Facilitator. In this position, you will be working  closely with children, biological parents and caregivers to deliver, implement and monitor the services offered through the access and visitation department.You will contact and interact with individuals/families referred or identified for services, arrange for service referrals, monitor and assure that services are being provided. Maintain schedules and arrange for appointments for the provision of visitation services and supervise parent(s) and child(ren) in their adjustment to the visitation.
